In order for us to pay you, the United States requires that we collect your W-9 form (or, if you do not have a U.S. tax ID, your W-8BEN form).  Round Table Group also recommends that you sign up for Electronic Funds Transfer, though that isn’t required. In order to protect the privacy of your personally identifiable information, we’ve moved all of these forms online.  

Government Documents

If you are a U.S. citizen, resident or business and have a pre-filled W-9, you can upload it here.  Otherwise, fill out and sign the W-9 online here. If you are not a U.S. citizen, resident or business, please fill out a W-8BEN form here (see the IRS website for W-8BEN instructions). If you have a pre-filled W-8BEN, please upload it here.  

Direct Deposit

If you wish to have Direct Deposit, you can securely fill out our Electronic Fund Transfer form.  Please note: it will ask you to upload a voided check, bank letter or deposit slip so that we can confirm your banking information. (We don’t want to send your money to someone else!)  

Non-U.S. (International) Bank Information

If you use a bank outside of the United States and would like us to deposit your funds into that account, please fill out our International Banking Instructions form. If you already filled out the form and want to upload it, please do so here.   Once you have finished and submitted each form, it will take you to a “complete” page. Thank you again.
